REPAIRS:
While
under the one year Warranty Period - return with a
copy of the original lnvoice and ship via
Priority, insured and to the above LIGHTFORMS address. Include
a check for $15 for return shipping and handling. Repairs are accomplished with in a week, usually the same
day, and returned without charge. (to return via UPS
or another shipper other than the United States Postal
Service - call for shipping address)
After
the one year warranty period, ship to Lightforms (same as
above).
BULBS last a very long time, however if there is a
circuit board or bulb problem, Both are replaced for
$75. Repairs to any other components are charged
accordingly and the cost of return shipping is added
to the invoice.
